HIPAA Forms Privacy Statement

The Women's Center is concerned about your privacy. We have created forms that ask for personally identifiable information -- among other things, your name, phone number, and address. If you choose to sign up for Women's Center services using an online form, then you agree to the collection and use of information as outlined below. Information Collection and Use

The information that we collect will be used to:

  • contact or identify you;
  • connect you with the services you're interested in; and/or
  • ask for feedback.
  • We will not use or share your information with anyone except as described in this Privacy Policy.

    Log Data

    Whenever you visit our website, we collect information that your browser sends to us that is called Log Data.
